FOR THE 2019/2020 SCHOOL YEAR:

Athlete/Parent/Guardian Concussion Information Sheet and Acknowledgement Form

Attention students wishing to participate in interscholastic athletics at Abington Senior and Junior High School during the 2018 -2019 school year: To participate on a sports team, students must have a comprehensive initial pre-participation physical evaluation – “CIPPE” – performed by an Authorized Medical Examiner.

PIAA 2019-20

Physicals should be completed on or after June 1, 2019 and will be effective during the school year, through May 31, 2020.

It is mandatory that students submit their PIAA (including immunization) as soon as possible after May 31st, preferably by July 12th if they want to participate in any sport activity in the fall. 

When school is in session, PIAA forms are submitted to the health suite where the child attends.  Forms submitted during SUMMER MONTHS:

  • 9th-12th grade to Senior High Receptionist. .
  • 7th and 8th Grade sports to Junior High Receptionist.

 

IMPORTANT: If a student (Grade 9 – 12) has not provided the PIAA forms prior to July 12th, PLEASE inform them of the following:

  • Wednesday, July 31st – SR HI FOOTBALL ONLY report to the JUNIOR HIGH Health Suite at 9:00 A.M. with completed 6-page PIAA form and a copy of current immunization record.
  • Monday, August 5th (9th – 12th grade) Fall sports participants report to the JUNIOR HIGH Health Suite with completed 6-page PIAA form and copy of current immunization record according to the schedule below:

9:00 AM – Soccer (Boys)

10:30 AM – Girls Soccer

9:45 AM – Field Hockey

10:40 AM – Girl’s Volleyball

10:10 AM – Girl’s Tennis

10:50 AM – Golf

10:20 AM – Cross Country (Girls & Boys)

11:00 AM – 9th Grade Football

 

11:40 AM – Cheerleading

 

Freshman Football and Freshman Boys Soccer 2019 Information

  • PIAA Forms for during SUMMER MONTHS to Senior High Receptionist. 
  • Monday, August 5th  ONLY:  Report to the Junior High School Health Suite with completed 6-page PIAA form and copy of current immunization record at TBA
  • Practices begin Monday, August 19, 2019 with time to be determined
  • Freshman Football Heat Acclimatization = August 19, 20, 21
  • Freshman Football First Scrimmage – Thursday, September 5
  • Boys Freshman Soccer First Game – Monday, September 9
  • Freshman Football First Game – Thursday, September 19

7th/8th Football 2019 Information

  • PIAA Form due to Health Suite by Tuesday, September 3, 2019 *Physical Must be dated 6/1/2019 or after.
  • Mandatory Football Conditioning – September 4 and 5
  • Football Pad and Helmet Fitting – Friday, September 6, 2019
  • 7/8 Football Heat Acclimatization – September 19, 10, 11
  • First Full Pads Practice – Monday, September 12, 2019
  • Scrimmage – Wednesday, September 25th, 2019

Fall Sport 2019  Information – Boys 7/8 Soccer, Girls 7/8 Soccer, 7/8 Field Hockey, Girls 7/8 Tennis, and Girls 7/8 Volleyball

  • PIAA Form due to Health Suite by Friday, September 6, 2019  *Physical Must be dated 6/1/2019 or after. **Only hand in if playing a FALL Sport
  • Information Meetings – Early September
  • Try-outs start Tuesday, September 10th

Students cannot practice with the team if they have not been cleared and processed.

Student, parent, and Authorized Medical Examiner’s signatures are required on the PIAA CIPPE form.
