FORMS AND RESOURCES

Step by Step College Process – this document will guide you through the steps of applying to college, requesting transcripts, and letters of recommendation.

Transcript Release Form – this form must be completed in order for transcripts to be sent to college, universities, and scholarship programs.

Midyear Transcript Request Form – this form must be completed and returned to your school counselor if midyear transcripts are required by your college or university.
